What are the responsibilities and job description for the UIR Coordinator position at Occupational Training Center?
- Successfully completes any additional updated DDD Investigation Training when applicable.
- Reviews and submits initial and follow up UIR reports to DDD/CIMU for all qualified incidents.
- Reviews all GERs to ensure all reportable incidents are submitted through UIR documentation.
- Ensures all program UIRs are submitted on time and have complete and accurate information before being submitted.
- Oversees the NJIRMS UIR system to unsure all incidents are being address and all follow-up are submitted within 30 days (or other assigned due date).
- Assigns all UIR Investigations to trained investigators, provides them with the necessary UIR information and give them an initial list of people who must be interviewed.
- Completes investigations as needed.
- Trains new investigators on how to complete investigation paperwork.
- Manages and monitors OTC Services’ UIR tracking system to ensure all incidents are documented and follow up information is being obtained and submitted.
- Reviews all Investigations and when accurately and thoroughly completed, sends to the Director of Professional Services for final review and sign off.
- Provides follow up and remediation of deficiencies discovered during the investigation. Ensures additional training or disciplinary feedback is completed as a result of the investigation outcome.
- Sends follow-up written communication to appropriate parties when investigations are concluded.
- Serves as the lead contact for DDD and CIMU. Coordinates and facilitates all Investigation follow up meetings if needed.
- Prepares Quarterly UIR Reports for the QI committee meetings.
- Conducts annual and re-training (when necessary) for staff members on Abuse, Neglect and Exploitation.
- Conducts annual training for supervisory and vocational staff on UIR documentation.
- Participates as a member of OTC Services’ Safety Committee to provide input on systemic issues and make recommendations for changes.
- Maintains the highest level of confidentiality and integrity regarding all UIR and investigation information.
