Education, Experience, Minimum Qualifications:

  • Master's degree in social work required from an accredited institution.
  • Two (2) years of recent, relevant hospital or clinical experience preferred.
  • Must have a National Provider Identifier (NPI), or obtain an NPI, before the start date.
  • Experience mobilizing community resources, navigating through the healthcare continuum, and working with disparate populations.
  • Licensed as an LCSW (Licensed Clinical Social Worker) in the State of Washington.
  • Current Basic Life Support (BLS) certification.

Job Summary:

Applies social work methods and theory to address psychosocial impairment or disability, including mental, emotional, and behavioral disorders. Provides direct assessment, treatment planning, and intervention under appropriate supervision as required by state regulations. Participates as a member of an integrated primary healthcare delivery team to provide coordinated, high-quality care. Acts as a liaison between clinical departments/disciplines and maintains a positive work environment that promotes OBHMC values.

Required Knowledge, Skills, Abilities:

  • Advanced knowledge of social work and behavioral health theories and practices. Demonstrated competence in clinical skills within scope of practice.
  • Ability to assess patient needs and exercise judgment in managing and coordinating care within regulatory guidelines.
  • Ability to cultivate effective partnerships and work in collaboration with providers, other health care professionals, caregivers, and patients. Strong leadership, facilitation, delegation, and coaching skills.
  • Proficient in the use of current technology, including Microsoft Office products. Ability to learn and effectively use electronic medical records and other systems and equipment.
  • Professional and effective written and verbal communication skills. Ability to maintain accurate documentation and records.
  • Ability to work independently within the limits of licensure and in collaboration with supervising providers when required.
  • Strong leadership, organizational, and analytical skills.
  • Ability to manage multiple priorities and display flexibility in a fast-paced and changing work environment.

Performance Responsibilities:

  • Performs collaborative behavioral health assessments and treatment planning for patients, ensuring compliance with LCSW scope of practice in Washington state. Collects data from various sources, including patient records, interviews, observation, and team members. Documents patient assessment and progress appropriately, maintaining timely, complete, and accurate records.
  • Works as a member of the integrated primary care team. Contributes to team-based care through effective partnerships with patients, caregivers, and other providers. Refers cases requiring independent diagnosis and treatment planning to appropriately licensed professionals, as necessary.
  • Provides immediate crisis intervention, behavioral health intervention, and support for social service-related issues in collaboration with the care team. Participates in and monitors tracking systems for identified patients. Coaches patients and families toward successful self-management.
  • Facilitates patient/family navigation through the healthcare system for assigned patients. Communicates effectively with primary care providers, hospitals, specialists, and post-acute care facilities to schedule appointments. May facilitate access to appropriate primary and specialty providers as well as other care coordination team support specialists (e.g., Diabetes Educators, Registered Dieticians). Follows up to ensure patients attend scheduled appointments.
  • Serves as the point-of-contact, advocate, and informational resource for assigned patients, family, care team, payers, and community resources. Proactively acts as a patient advocate, responding with empathy and respect to resolve patient/family concerns.
  • Builds and maintains effective working relationships. Serves as a liaison across disciplines. Participates as an active member of committees; leads workgroups, projects, and committees as assigned.
  • Maintains professional skills and awareness of regulatory changes, compliance issues, innovations, and developments through research and continuing education opportunities. Stays abreast of trends and best practices.
  • Performs special projects and other related duties as assigned.

  • Participates in departmental and functional strategies and tactics to enhance patient care and overall organizational performance.
