POSITION SUMMARY : - Assists with the planning, organization, development, coordination and direction of the Nursing Department in its Programs and activities in accordance with current Federal, State and Local guidelines and regulations, accepted standards of practice and as may be directed by Administration and / or Director of Nursing

JOB FUNCTIONS :

QUALIFICATIONS :

Registered Professional Nurse.

Bachelor of Science in Nursing or related health care field preferred.

Previous supervisory experience of at least 2 years in a long term care facility preferred.

PRI certification a plus.

Knowledge of MDS and Care Plans.

IV certification a plus.

THE FOLLOWING DUTIES / RESPONSIBILITIES ARE ESSENTIAL FUNCTIONS OF THE JOB THAT MUST BE SATISFACTORILY MET WITH OR WITHOUT ACCOMMODATION

DEPARTMENTAL :

Assists with the planning, organization, development, coordination and direction of the Nursing Department in its Programs and activities in accordance with current Federal, State and Local guidelines and regulations, accepted standards of practice and as may be directed by Administration and / or Director of Nursing

Directly supervises assigned staff which includes but is not limited to, participating in orientation, manage staff performance in relation to the position’s standards, and  monitor time and attendance.

Exercises good independent judgment and discretion in effectively managing assigned staff by, but not limited to, conducting performance appraisals, corrective disciplinary action and other supervisory interventions which may affect a change in the assigned staff’s status of employment.

Makes recommendations to Department Head or designee, as to the hiring, firing, advancement, promotion or any other change in employment status.

Supervises and directs the activities of all nursing personnel on their shift to assure the provision of quality care in a safe and therapeutic environment. Makes rounds at least twice per shift to each unit assure same and assess and residents, staff, or other pertinent issues.

Responsible for the evaluation and monitoring of all levels of resident care through on-site observations and audits, including the evaluation of Interim Care Plans for quality, appropriateness and effectiveness.

Oversee / coordinate the tasks of the Night Supervisors and other assigned staff.

Assures through supervision, monitoring and auditing that all responsibilities of assigned staff’s job descriptions are fulfilled.

Responsible to identify educational needs.

Inservice staff in general staff meetings, specific unit meetings and individual meetings as necessary.

Identify staff issues; ensure compliance to standards, policies and philosophies. Institute disciplinary measures as appropriate. Notify DNS / ADNS.

Conducts regular meetings with staff.

Carries out major assignments in conducting the clinical care operations of the business.

Has authority to negotiate and bind the facility on significant matters.

Investigates and resolves matters of significance on behalf of management.

Represents the facility in handling complaints, arbitrating disputes, or resolves grievances.

The RN Supervisor’s responsibilities have a significant impact on the clinical care operations of the facility.

RN Supervisor has the authority to formulate, affect, interpret, and implement management policies and operating practices with regard to Nursing responsibilities.

RN Supervisor responsibilities affect facility operations to a substantial degree.

Participates in the development, interpretation and implementation of the nursing department’s philosophy, objectives, standards, policies and procedures.

Responsible for coordination of the activities / resident care of the evening / night shift.

Has the authority to initiate, coordinate and participate in all investigations evaluations of accidents / incidents such as, but not limited to :

Responding and evaluating all accidents / incidents.

Signing off on Accident / Incident reports.

Notifies ADNS / DNS of injuries of unknown origin.

Directs nursing staff in the correct completion of Accident / Incident forms.

Initiates and participates in investigation of injuries of unknown origin.

Ensure that the physician sees all residents who have an incident / accident.

Ensures that the resident is requested to state his / her version of the accident / incident, and he / she or designated other is given a copy.

Checks all Accident / Incident forms for completeness accuracy. Inservices staff as necessary to complete forms correctly.

Ensures that all follow-up documentation required is completed by the licensed nurse.

Ensure care plan is updated on the shift in which the accident / incident occurred.

Ensures all necessary follow-up is initiated and provided to the resident.

Emphasizes through on-site monitoring and direction adherence to residents’ rights in all aspects of care.

Ensures that documentation on 24 hour report is accurate / thorough and follows through on all residents listed on the 24 hour report.

Monitors all residents on oxygen, tube feedings, suctioning, pressure ulcer care protocols, and behavioral problems, and any other residents’ needs.

Emphasizes through direction, monitoring and auditing the importance and requirements of Infection Control in all aspects of care. Ensures isolation technique is maintained. Emphasizes universal precautions.

Observes and assist in Dining Room during resident meal times and ensures that all staff are present and performing their dining room assignment.

Ensures that nursing staffing patterns and assignments scheduled are maintained :

Provides appropriate relief staffing when needed.

Assures that resident services rendered by nursing staff are not interrupted.

Supervises and monitors the assignment of staff breaks and meal time to assure continuity of care provided.

Each supervisor is responsible to review the daily schedule.

Participation in QA program through completion of audits and developing / implementing corrective action.

Attend seminars, conferences, etc., as deemed necessary.

Ensure through observation and documentation audits, that all residents with pressure ulcers have documentation following the specific pressure ulcers protocol.

Ensures resident chart, forms, and other documents are prepared for the physician visit.

Ensures any orders written by the physician have been noted and transcribed correctly through audits.

Facilitates continuous improvement for staff via conferences, seminars and professional groups to stay current with changes in long term care.

Serves as a role model in professionalism, conduct, appearance and continuous improvement.

Reports daily to the DNS / ADNS / RN Manager on all pertinent issues and maintains open communications with the DNS / ADNS / RN Manager on all issues relevant to residents and staff.

Responsible for coordinating and assisting in all resident and staff emergencies; assures that emergency procedures are followed through; reports to ADNS / DNS / Administrator-On-Call where appropriate.

Communicates with RN Managers and assures continuity of care to all residents as per RN Managers’ instructions and resident care plans.

Communicates all issues relevant to the unit / residents to RN Manager.

All other duties as directed by DNS / ADNS.

Keeps inventory, order, and maintain supplies needed for evening / night shift.

Performs other duties as assigned.

FACILITY-WIDE :

Must possess the ability to consistently come to work as assigned, including coming to work regularly and on time.

Must possess the ability to adhere to the time recording policy, including adhering to meal / break policy / schedules, as assigned.

Must comply with departmental dress code requirements including prominently wearing one’s ID card, and maintaining normal levels of personal hygiene and professional appearance.

Must have the ability to satisfactorily meet work performance and productivity standards as they relate to the position.

Must participate in and adhere to facility programs and activities such as but not limited to, employee health, safety, security and corporate compliance.

Must participate in facility mandatory in-service and job training programs and activities, as assigned.

Must perform all duties and all interactions in accordance with current Federal, State, Local guidelines and regulations, Facility policies and procedures, and accepted standards of practice.

Must have the ability to uphold the highest standards of ethics, integrity and honesty in matters of conduct and behavior as it pertains to work performance, position and profession.

Must have the ability to effectively and professionally cope with and manage interactions with patients / residents and their family members and / or visitors, as well as, co-workers, to ensure the best possible care, customer service and satisfaction.

Must have the ability to maintain confidentiality in all matters concerning Horizon Care Center resident / patients as well as assigned computer / software and work product.

In an emergency, may be asked to perform other functions as deemed necessary.

JOB COMPETENCY :

Must maintain and timely provide facility with current license / certification as required by the position.

Must be able to read, write, speak and understand the English language with sufficient proficiency to meet the essential functions of the position.

Must possess the ability to utilize critical thinking skills and make good independent decisions when circumstances warrant such actions.

Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.

Must function independently, demonstrate flexibility and have the ability to handle multiple tasks simultaneously.

Must be willing to seek out new methods and principles and be willing to incorporate them into existing Nursing Department practices.

Must demonstrate ability to maintain satisfactory competency levels and professional standards of practice.

Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the essential functions of this position can be fully met.

Must have the ability to operate a computer or use prosthetics or devices that will enable such use.

Must be physically able to operate Horizon Care Center specific equipment in relation to the position or use prosthetics or devices that will enable such use.

SETTING :

Must be able to perform duties throughout Horizon Care Center.

Must be able to sit, stand, walk, climb, stretch, and bend with sufficient ability to meet the essential functions of the position.

Must be able to stand, lift, push and pull at least 50 lbs

Must be in good general health and demonstrate emotional stability with sufficient proficiency to meet the essential functions of the position.

May be subject to frequent interruptions

May be subject to hostile and emotionally upset residents, family members, etc.

Must be able to work beyond normal working hours, on weekends, and in other positions temporarily, when necessary.

Is subject to stay or call back during an emergency (such as but not limited to severe weather, utilities failure, evacuation, disasters etc)
