Certified Occupational Therapy Assistant (COTA)/Independent Living Coordinator

 

Reports to: Home Access Program Manager


Job Description: The Certified Occupational Therapy Assistant (COTA)/Independent Living (IL) Coordinator will assist individuals with disabilities by enhancing their quality of life through access to Independent Living services. Independent living coordinators provide support to individuals by reducing barriers to independence by acquisition of appropriate assistive devices, technologies, and home modifications.

Duties/Responsibilities:

·      Conduct comprehensive assessments to identify the independent living (IL) needs of individuals with disabilities, establishing priorities for goal achievement and service provision. 

·      Conduct home assessments, including physical measurements, to evaluate the individual’s living environment and identify modifications to increase accessibility and safety.

·      Evaluate Fee-for-Service (FFS) consumers referred by insurance companies to determine eligibility and needs for assistive devices and home modifications.

·      Conduct research to identify the most cost-effective assistive technology products and solutions that increase access and independence for consumers.

·      Collaborate with vendors and insurance companies to ensure that recommended equipment is covered under the consumer’s insurance plan.

·      Ensure effective coordination and communication between all stakeholders to facilitate the timely provision of recommended equipment.

·      Participate as an active member of an interdisciplinary team, working collaboratively to identify and address barriers to independence for individuals with disabilities.

·      Perform any additional tasks or responsibilities as needed or directed by management.


Required Education, Experience, and Qualifications:

·     Current certification as an Occupational Therapy Assistant (COTA)

·     In-depth understanding of assistive technology devices and home modifications available to individuals with disabilities

·     Strong organizational skills with the ability to manage time effectively and coordinate multiple tasks and schedules

·     A valid driver’s license and access to a reliable vehicle for conducting home visits

·     Proven experience in addressing the needs of individuals with disabilities, access to necessary services and fostering independence

·     Solid knowledge of the barriers faced by individuals with disabilities both within their homes and in their community

·     Exceptional written and verbal communication skills, with the ability to accurately document and convey consumer needs and challenges

·     Ability to work independently, set priorities, and manage tasks to meet deadlines effectively

·     Proficiency in Microsoft Office Suite (Word, Excel, Outlook), with a general aptitude for using software and technology in a professional setting

·     Strong interpersonal skills with the ability to develop and maintain professional relationships with colleagues, insurance providers, and vendors

·     Capacity to advocate for consumers and educate community members, professionals and stakeholders on the variety of services that OSCIL provides to fulfill the mission of Independent Living

Salary range: $24-$26 per hour

Hours: 35 hours per week
