The person in this position is responsible for the direction, oversight and coordination of Oceanview’s wellness and care management programs including independent resident transition to assisted living. They also serve as the Administrator for Preferred Retirement Options (PRO), OceanView’s personal care agency. In this role, they are responsible for the regulatory compliance of the organization, the hiring, training, ongoing assessment of staff performance and completion of performance appraisals. They work collaboratively with the Director of Operations in the development of departmental goals, objectives and annual budgets; provides regular reports of activities while ensuring ongoing communication in a timely manner relative to any issues or concerns that arise. Generally, the regular business hours for this position are 8:30AM-5:00PM, Monday – Friday, however flexibility in schedule is necessary from time to time in order to meet resident and company needs. Duties include and are not limited to:

  • Providing healthcare management services for independent living residents facilitating coordination of care while promoting independence; maintaining ongoing communications with residents and their families throughout the continuum of care
  • Assisting residents and their families when a transition to assisted living is in the best interest of the resident; working closely with the resident’s physician and family to obtain all required documentation to ensure a smooth transition; keeping the Director of Assisted Living and Sales Associate informed of progress
  • Working with residents and their families to help them understand their long-term care policy benefits and assisting them in the initiation of their claim for home care services and/or assisted living as requested
  • Developing, revising, and implementing resident wellness and education programs
  • Providing oversight for recruitment of new employees, hiring qualified staff with goal of

retention, ensuring recruitment, pre-employment and new hire processes are followed

  • Coaching, counseling, praising staff accordingly along with other necessary performance related feedback such as disciplinary action
  • Reviewing existing policies, procedures and protocols periodically to be certain they are current, accurate, and complete; updating as needed
  • Completing all required reports in a timely manner including, and not limited to: Bi-weekly payroll Monthly Wellness / PRO Report Monthly Case Management and PRO Billing Introductory and Annual Performance Appraisals Budget Variance Reports upon request
  • Interacting positively with external customers contributing to their success within the OceanView community
  • Developing strategies and promoting PRO services in the community to reduce the use of outside agencies
  • Providing a positive and productive work environment by promoting a positive image and collaborative working relationship within and between all departments; participating in management and marketing team meetings providing updated resident information, PRO information as needed
  • Proactively ensuring a safe environment for residents and staff via ongoing observation of resident’s personal space and the OceanView community environment for potential risks and hazards; reporting hazards to appropriate department for mitigation
  • Facilitating ongoing professional development for self and staff via staff meetings, attendance at in-service programs and off-site learning programs
  • Ensuring confidentiality of resident information as well as proprietary company information

EXPERIENCE

Minimum of three years’ experience as a Registered Nurse with two years’ experience in a geriatric or home care setting.

A minimum of two years clinical management experience is required. Must possess excellent clinical assessment skills, critical thinking and problem-solving skills, excellent interpersonal skills. Excellent verbal and written communication skills and good command of the English language required.

Pay: $82,000.00 - $86,000.00 per year
