This job description, defining qualifications, primary (essential) and secondary (non-essential) functions, responsibilities and duties, is intended as a guideline for the employee and manager. The duties listed here are not considered to be all inclusive. The employee in this position performs these and other duties as required, although not listed, which may be assigned by his supervisor or facility Administrator / designee.
ESSENTIAL FUNCTIONS :
Ensures accurate and timely completion of all resident assessment instruments including MDS sheets, face sheets, trigger sheets, RAP summaries, and care plans.
Teach and in-service new and present employees in the process and laws governing resident assessment.
Update staff on new and / or revised laws and interpretations of laws governing resident assessment.
Upon a resident's admission, log patient in on appropriate calendar and assist in initiating the assessment process.
Assure that the Minimum Data Set (MDS) is appropriately completed and signed by each discipline before the end of each patient's assessment schedule.
The coordinator will :
1. Sign that the MDS is complete.
2. Complete nursing sections of the MDS or assist as needed.
3. During the 14-day assessment following admission, circle / highlight appropriate triggers and pull corresponding RAPs for completion.
4. Complete nursing RAPs or assist as needed.
On or before day 14, conduct an informal team meeting to discuss care planning for that resident.
Oversee care planning from day 14 through completion on day 21, assisting nursing on an as-needed basis.
Sign the back of each trigger sheet once each trigger has been appropriately addressed, a statement made on each regarding to location of information, and date has been addressed.
Sign quarterly assessments when completed and provide appropriate follow through on care plans overseen.
Assure that residents with outstanding care plan needs upon admission have those needs addressed when appropriate rather than holding these until completion of the MDS.
Assume the same responsibilities with all Minimum Data Sets started secondary to patient change in status and all annual Minimum Data Sets.
Report on continuing quality assurance and improvement activities to CQI committee as needed.
Electronically transmits per MDS assessment schedule, but at least monthly.
Tracks submissions / discharges / re-entry's / status changes using logs.
Completes reports required.
Tracks consistency between documentation.
Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.
NON-ESSENTIAL FUNCTIONS :
Assists Director of Nursing with department quality assurance as needed.
May perform any and all staff nurse or supervisory duties as required in emergency situations.
Prerequisites :
1. Registered Nurse licensed by the Commonwealth of Massachusetts or eligible
2. Two years previous experience in long term care. Knowledge of DPH, OBRA, Joint Commission, Medicare regs and MDS process.
3. Experience working within an interdisciplinary team approach to care.
4. Computer skills.
