MAJOR FUNCTIONS:
The Case Manager is responsible for substance abuse counseling and intervention services. Must be able to stabilize and promote self-sufficiency and optimum health for people living with HIV/AIDS, Mental illness and/or substance use disorders. Manage a case load of 20 to 25. Provide advocacy as well as coordinate services through home visits.
MAJOR FUNCTION:
To provide effective and efficient advocacy service as part of the induction process including program interviews, ascertaining information and orientating prospective residents in the Odyssey House admissions process.
SPECIFIC DUTIES & RESPONSIBILITIES:
1. Ensure potential inductions are serviced immediately in a professional and concerned manner.
2. Make sure all documents are included in new charts prepared prior to transfer.
3. Responsible for own active files making sure they are updated, accurate, secured and that all documents are recorded and filed correctly.
4. Submit reports as required
5. Counsel, screen, interview, and induct residents.
6. Obtain medical and psychiatric clearances when necessary.
7. Participate in outreach engagements.
8. Develop files on new inductions.
9. Coordinate individual inactive charts/closure.
10. Other projects and assignments as required.
11. Follow up with scheduled interviews/inductions.
REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE SKILLS AND ABILITIES.
High School Diploma or equivalent with minimum of one year experience in a TC and/or admissions knowledge and experience. CASAC/CASAC-T
