MAJOR FUNCTIONS:
The case manager provides case management services to tenants who have a history of chronic street homelessness and substance abuse issues. The case manager oversees the clinical aspects of the program, including intake & assessment, client engagement, treatment and service plans, and coordination amongst other providers for service needs (i.e., day treatment, psychiatric care, etc).
SPECIFIC DUTIES & RESPONSIBILITIES:
- Provide case management to assigned caseload.
- Ensure service delivery and documentation compliance in accordance with the written agreements and contracts.
- Participate in tenant selection, intake and orientation.
- Work as a member of the social service team in the development of on-site and off-site socialization, vocational/educational, risk reduction and skill-building activities with a view toward community inclusion and integration.
- Formulates appropriate Vocational & comprehensive written service plan within 30 days of admission and service plan review every quarter.
- Conduct Mental Health Assessments for all tenants.
- Refer tenants, as needed for appropriate mental health services.
- Serve as liaison among community realtors, and groups including, social service, government, and business organizations.
- Participate in ongoing individual supervision, team meetings, case conferencing, division meetings and trainings, internal and external.
- Update the apartment furniture equipment registers annually.
- Adhere to on-call systems to facilitate crisis intervention, emergencies and other situations that require immediate action.
- Maintain program participant files in accordance with funding regulations.
- Conduct or arrange annually and monthly housing quality inspections of housing units occupied by program participants. Identify deficiencies in maintenance and communicate with program participants and/or landlords, as appropriate, to correct deficiencies.
- Conduct or arrange for inspections annually, complete recertification in accordance with regulations.
- Intervene and deal with problems from landlords and program participants; assists in the resolution of these problems.
- Develop and maintain good landlord-tenant relations among program participants.
- 24 hour/ 7 day on call staffing availability.
- Collection of all tenant rent.
- Other duties as assigned.
*Requirements: CASAC or CASAC-T with 2 years of related experience. Knowledge of mental illness, substance use, HIV/AIDS, and homelessness. *
