- Responsibilities :
- Provide pharmaceutical care by overseeing the medication process, identifying and preventing drug-related issues, and resolving existing problems.
- Perform all duties expected of a registered pharmacist as per Texas Children's Hospital standards.
- Tasks will be performed according to the developmental needs of patients, ranging from infants to adults.
- Ensure the appropriateness of medication doses considering the patient's diagnosis, weight, allergies, and potential drug interactions. This is measured by a lack of medication-related incidents.
- Accurately input medication orders in the system, with no medication incidents.
- Confirm the accuracy of medications dispensed, with no errors.
- Ensure medication orders are processed within guidelines, as indicated by peer reviews or lack of complaints or incidents.
- Follow strict aseptic and infection control procedures when preparing sterile products. Ensure all team members, including technicians and other pharmacists, adhere to these standards. Complete annual re-certification and score at least 90% in skills and written exams, verified by a qualified observer.
