Become a trusted home and community based provider supporting local clients in Habilitation, Respite and/or Attendant Care through DDD/AZDES. We also have lots of opportunity to grow! This subcontractor position (1099) is a great opportunity for supplemental or primary income for anyone with a desire to enrich the lives of others.
The ideal person will have had experience in working with people with developmental disabilities.
Minimum Qualifications:
CPR & First Aid
Article 9
Level 1 Fingerprint Clearance Card or able to obtain one
Ability to pass background checks (E-Verify, Central Registry, etc.)
Valid driver's License

Schedule: Monday-Friday 3pm-8pm Weekends as needed for RSP

Scope of Work
Key Responsibilities:
Personal Care:
- Assist individuals with daily living activities such as hygiene, grooming, bathing,
- toileting, and dressing.
- Provide medication reminders and assist with mobility as needed.
Community Integration:
- Support individuals in participating in community activities, volunteering, or
- attending appointments.
- Facilitate social engagement by encouraging interactions and building relationships.
Skill Development:
- Teach and reinforce skills such as cooking, budgeting, cleaning, and other
- independent living tasks.
- Support individuals in achieving their personal goals as outlined in their Individual
- Support Plan (ISP).
Transportation:
- Provide safe and reliable transportation to appointments, recreational activities, or
- community events as required.
- Maintain proper documentation of mileage and travel if applicable.
Documentation:
- Accurately document services provided, progress toward goals, and any incidents.
- Submit required reports, timesheets, and other documentation in a timely manner.
Advocacy:
- Respect and advocate for the rights and dignity of individuals served.
- Respond to individual needs and communicate concerns to the appropriate program
- staff.
