Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in providing support and assistance to individuals facing various challenges, particularly in the areas of housing instablity, transportation, mental health, substance use disorder, trauma or other. This role requires a commitment to enhancing the well-being of clients through effective care plans, advocating, support through the legal process. Assist clients in finding assistance through community programs.
Summary of Duties:
- Identify individual client needs such as housing instability, transportation, mental health, substance use disorder, trauma, or other
- Provide critical support to attorneys by addressing the social, psychological, and environmental factors impacting clients
- Develop individualized care plans for clients to connect those clients with appropriate community resources and providers
- Advocate for alternatives to incarceration such as diversion or rehabilitation programs
- Support clients through the legal process to promote fair and just outcomes
- Assist clients in learning how to access community resources and services
- Work with attorney to coordinate community-based housing and treatment options for clients on conditional release plans
- Maintain and update electronic client files
- Make client referrals for services to other agencies as appropriate
- Maintain liaison with community agencies and service organizations to connect clients with those community services
Qualifications:
- Experience preferred but not required
- Good knowledge of community resources
- Ability to assist clients in accessing community resources and services
- Ability to communicate effectively
- Ability to appraise and organize facts to present materials in written and oral form
- Ability to establish and maintain effective working relationships with office staff, clients, and the general public
- Ability to manage a large caseload while maintaining adequate levels of communication and attention to individual clients
- Proficient computer skills
- Bachelor's degree in social work or a related field preferred
