Assisted Living Caregiver
Key Responsibilities:
● Assist residents with Activities of Daily Living (ADLs), including bathing, grooming, dressing, toileting, mobility, incontinence care, and eating.
● Participate in shift change routines, including reviewing shift reports, communication logs, and walking rounds.
● Administer medications according to AZDHS guidelines and Ohana policies (must be medication certified).
● Prepare, serve, and assist with meals following dietary plans and individual needs to support nutrition intake.
● Provide companionship, emotional support, and encouragement to residents throughout the day.
● Maintain a clean, safe, and organized environment including laundry, room tidiness, and shared living spaces.
● Routinely monitor resident rooms, clothing, and hygiene needs; ensure dignity and comfort in all areas of care.
● Maintain accurate daily documentation on resident care, medication administration, behavior, and observations.
● Promptly report any concerns, incidents, or changes in resident condition to the Manager.
● Follow all AZDHS and facility policies regarding infection control, safety, emergency response, and resident rights.
● Attend scheduled training, team meetings, and performance check-ins.
● Perform other duties as assigned by the Director or Manager.
Qualifications:
● Certified Caregiver License (Arizona Department of Health Services) – Required
● CPR and First Aid Certification – Required
● AZ Fingerprint Clearance Card – Required
● High school diploma or GED equivalent
● Ability to lift, assist, and transfer residents safely
● Strong communication skills, teamwork mindset, and empathy
● Must maintain professionalism and respect for resident privacy at all times
