- Coordinate and manage discharge planning activities in compliance with federal and state regulations.
- Meet with residents / guests and their families within 72 hours of admission to identify discharge goals and preferences.
- Collaborate with the healthcare team—including providers, licensed nurses, therapists, and social workers—to develop and implement comprehensive discharge plans.
- Facilitate timely referrals to external facilities, providers, and community resources to ensure continuity of care.
- Assist in arranging home health services and necessary medical equipment to support a safe and efficient discharge process.
- Educating residents / guests on medication management, treatment or wound care, and other necessary nursing care for a safe transition.
- Monitor residents / guests progress throughout their stay, adjusting discharge plans as needed to meet evolving clinical and social needs.
- Graduate of an accredited school of nursing.
- Current license as a Registered Nurse in the State of Hawaii required.
- 4 or more years’ experience in restorative and gerontological nursing.
- Discharge planning experience, preferred.
- Strong communication and interpersonal skills preferred.
- Ability to effectively multitask and coordinate care in a fast-paced environment preferred.
