Ohel is seeking a Licensed Mental Health Clinician to work on a field-based innovative team that serves participants transitioning to the community from a higher level of care. This role will include facilitating participants' transition from higher care levels to the community, offering counseling, skill development, and ensuring connections with community providers.
Responsibilities:
- Engage in transition planning and needs assessment
- Monitor discharge and ongoing care planning
- Build collaborative relationships with community providers
- Provide short-term counseling and risk assessment
- Resolve clinical issues affecting housing stability
- Assist with appointment navigation and referrals
- Conduct comprehensive psychosocial assessments
- Monitor participant progress and goal achievement
- Conduct family conferences and psychoeducation
- Provide crisis intervention and support services
Qualifications:
- Masters degree in Social Work or Mental Health Counseling or a related field required (LMSW, LCSW, LMHC)
- Knowledge of mental illness, substance use disorders, and NYC behavioral health programs. Knowledge of treatment and rehabilitation programs
- Knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques
- Ability to develop, evaluate, implement and modify a clinical treatment intervention to meet the needs of individual recipients.
The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills. This full-time position is located in Brooklyn. Hybrid schedule offered, with inpatient, home and community visits required. Position includes a competitive benefits package.
