What are the responsibilities and job description for the Mental Health Case Manager position at OHI?
The Mental Health Case Manager works with a Team in the community to support adults with mental illness to lead self-directed lives and contribute to their communities. This includes assessment and treatment planning; teaching daily living, communication, and work skills; coordination or assistance with medical or other appointments; and providing or coordinating safe transportation. Creation and maintenance of records and licensing compliance is also important.
The Mental Health Case Manager provides an essential role in assisting people to live in the community, reduce crisis situations, and lessen hospitalizations. Working cooperatively with community and medical providers, local law enforcement, and others is a must.
Job Type: Full-time
Pay: $23.00 - $23.50 per hour
