The Wound Ostomy Program Coordinator provides leadership related to managing and delivering quality, cost effective, patient and family centered care in the areas of Wound Ostomy and Continence Care across the critical access hospital and provider practice setting. S/he is an expert in the WOCN standards of practice. S/he is accountable for developing and leading programs for improvement in WOCN related outcomes. These include but are not limited to Hospital Acquired Pressure Injuries (HAPI), enhancing clinical nurses’ knowledge related to WOCN and HAPI prevention, and other outcome data surrounding wound and ostomy patient care as needed. S/he is dedication to continuous professional development staying up to date on best evidence guidelines, clinical pathways, and algorithms to assist in development, implementation, and monitoring of wound/ostomy care nursing care provided.
S/he works within an interprofessional environment that requires exceptional interpersonal skills, including the ability to establish and maintain effective relationships with patients, physicians, clinical/hospital nursing staff, leadership and support services. This role requires the use of specialized nursing skills to demonstrate patient assessments and nursing interventions to be implemented; the ability to use appropriate judgment, independent thinking, and creativity when working to identify solutions to patient care issues; and coordinate patient education for the population this role serves.
S/he is an expert clinician providing recommendations regarding managing and delivery of WOCN. The WOCN monitors the quality and success of the wound ostomy care program through surveillance, direct observation, chart review, one to one interaction, and/or collaboration with the multidisciplinary health care team, Administrators, or Manager. The WOCN contributes to the infrastructure of the wound/ostomy program acting as a liaison and resource to both clinical and hospital nursing staff. S/he actively participates in decision-making, teaching, mentoring, and collaboration with the interprofessional team. The role of the WOCN requires the involvement of key stakeholders and others involved or impacted by the program. Good customer service and professional communication skills are essential.
- Responsible for program planning and coordination of wound ostomy service within Ohio County Hospital and Specialty Clinic.
- Oversee and develop wound care services in adherence to regulatory standards and physician orders.
- Lead educational efforts for hospital/clinical staff on wound/ostomy care topics, as well as educating patient and family members on wounds and wound care.
- Maintain continuous education and stay updated on the latest wound/ostomy care techniques and advancements.

- Performs comprehensive initial and ongoing review of wound/ostomy patient care services provided, specialty equipment and service contracts, and supply utilization. Make recommendations regarding new policies and procedures to improve.

- Serves as a resource for clinical and hospital nursing staff for problem-solving and decision-making. Provides teaching to staff on both a formal and informal basis.

- Participates in decision-making, teaching, mentoring, and collaboration with the interprofessional team to advance the patient’s plan of care and achieve exceptional outcomes.
- Remains committed to wound and ostomy care from prevention practices to intervention according to evidenced-based wound/ostomy care for patients.
- Assists with the initiation and coordination of patient education
- Provide training and education regarding skin/wound assessments, ostomy markings and assessments for new and established ostomies.
- Provide guidance regarding etiology and pressure injury staging
- Support the patient care team in wound/skin breakdown-related prevention and treatment options
- Develop standard operating procedures for the proper storage and maintenance of wound vac equipment and supplies.

- Determine list of required ostomy care supplies for medical surgical unit of Ohio County Hospital. Identify par level for supply management and outline proper storage in both patient care and non-patient care areas.
- Develop training & demonstration of ostomy markings and assessments for both new and established ostomies
- Primary liaison between KCI rep and hospital/clinic staff.
- Coordinate with KCI to develop SOP for the use of wound vacs in both hospital and outpatient settings. Outline required paperwork, prior authorization, care coordination, and patient care congruent with contractual agreement and evidenced based practice.
- Assess storage of wound vac equipment and supplies for patient care. Develop standard operating procedures.

- Identify trends in nursing practice and establish plan with local leadership team to support quality patient care with pressure injury prevention
- Participate in prevalence survey and ongoing surveillance of hospital acquired pressure injuries.
- Collect and analyze pressure injury data, identify expected outcomes, and provide input on the development and implementation of an individualized plan of care
- Collaborate with clinical and support services and outside agencies or facilities in order to coordinate appropriate patient care
- Make recommendations regarding new policies and procedures designed to improve the patient experience
- Ensure that new policies and procedures are implemented in a timely manner
Qualifications
REQUIRED:
- · EDUCATION: Completion of accredited program for registered nurse (RN)
- · LICENSE: KY RN or Compact State License
- · EXPERIENCE: 3 years’ experience in wound/ostomy nursing.
- · OTHER: BLS
PREFERRED:
- EDUCATION: Bachelor of Nursing Degree (BSN)
- CERTIFICATION: Professional certification in wound ostomy nursing highly preferred
- EXPERIENCE: Experience with clinical quality, clinical operations, policy development and/or process improvement.
- SKILLS: The ideal candidate for this role has excellent verbal & written communication, demonstrate leadership skills, the knowledge of quality improvement processes, the ability to lead system change, and have experience building clinical networks. S/he will need to be able to effectively handle challenging situations, adapt to changes in work environment, and balance multiple priorities while working autonomously with minimal direction. S/he will have strong computer skills, including Microsoft Office, & Outlook. S/he should have experience with database entry, and the ability to analyze/utilize data, to support decisions.
