We learn, grow, and achieve more - in our careers and in our communities.Job Description Summary : Community Health Worker (CHW) will work with patients who are a part of our Clinically Integrated Network who are identified as having non-clinical barriers to engaging in treatment plans and recommendations. The goal of the CHW is to work with the multidisciplinary team to identify barriers to the patient's health care and coordinate support services and community-based resources to address those barriers with the patient. In addition, this position provides individual consultation and follow up based on Social Determinants of Health, patient demographics and lack of resources, and coordinates support for clients.This position will support our Independent Practices who are a part of our Clinically Integrated Network.
