Home Health RN Case Manager

Key Responsibilities:
- Provide ongoing assessment and nursing care to patients in homes throughout several counties.
- Develop care plans, provide treatment, and work closely with patients and families.
Requirements:
- Current Ohio nursing license
- One year of acute care or medical-surgical experience
- Experience in a Medicare-certified home health agency and knowledge of EMR preferred
