Job Description
We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Care Coordinator. The successful candidate will be responsible for planning, implementing, and evaluating patient care plans in collaboration with the interdisciplinary team.
Responsibilities:
- Assess and evaluate patients' physical, spiritual, and psychosocial needs.
- Develop and implement individualized care plans in consultation with patients and families.
- Review and update care plans regularly to reflect changing patient needs.
- Collaborate with other healthcare professionals to ensure seamless coordination of services.
- Supervise and educate Personal Care Specialists, Volunteers, and other caregivers.
Qualifications:
- Graduate of an accredited nursing school.
- Active RN license in the State of Ohio.
- Two years of acute care nursing experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred.
- Excellent communication and interpersonal skills.
