Title: Chaplain; PRN

This position is a driving position that will serve Washington County and the surrounding areas. You will be covering patient demand, PTO, and call-offs. Schedule will include shifts between Monday-Friday with hours of 8:00am-5:00pm. Schedule is flexible.

We provide superior care for patients in families in homes/facilities. Our Chaplain’s provide care grounded in initiating, developing, deepening and closing a spiritual and empathic relationship with those receiving care. The development of a genuine relationship is at the core of chaplaincy care. Our Chaplains make spiritual assessments of patients, considering mental, emotional, physical, and spiritual stresses, and respond with support, guidance and counseling. Chaplain duties pertain to the end of life needs of not only of patients, but also their families, caregivers, community, and even the interdisciplinary team.

Responsibilities:

  • Gathers and evaluates relevant information regarding the patient’s spiritual, religious, emotional and relational needs and resources.
  • Develops and implements a plan of care to promote the spiritual comfort of the patient.
  • Collaborates and participates as member of hospice team in regularly scheduled care conferences, interdisciplinary Team (IDT) meetings, and with other care providers to promote the understanding of spiritual needs of patients and / or families to ensure coordination of services. 
  • Models and collaborates with other care providers in respecting and providing sensitive care regardless of diverse beliefs, cultures or identities.
  • Consistently adjusts to meet the patient’s changing needs with sufficient visit frequency to continue to provide quality spiritual counseling and emotional support.  Productivity is monitored by manager.
  • Performs religious rites and ceremonies such as weddings, funeral services, baptisms and administers sacraments or arranges for them to be administered as requested.
  • Assists in developing and providing educational and public relations programs for staff and clergy community to ensure patients and families receive quality spiritual counseling and emotional support. 
  • Collaborates with Bereavement in facilitating support for individuals or groups.
  • Documents how the spiritual needs/concerns of the patient and /or families are addressed. Utilizes the plan of care to reflect goal/outcome and interventions.
  • Ensures quality communication with the patient’s Interdisciplinary Team by thoroughly documenting and updating assessments, goals and interventions in the electronic health record at the point of service and then synchronizing this documentation for each patient according to Ohio’s Hospice standards.

Qualifications:

  • Master of Divinity (M.Div.) or equivalent in Theology is required.
  • Preferred candidates are ordained or commissioned ministers that are ecclesiastically endorsed by their recognized faith group or religious organization.
  • Preferred candidates should be board certified, eligible for certification or working towards certification by APC, BC-HPCC, NACC or NAJC.
  • Clinical Pastoral Education (CPE) 4 units preferred; minimum 1 unit and enrolled for completion.
  • Three years of current active pastoral ministry or chaplaincy experience.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid Driver's License with Safe Driving Record.
  • State Minimum Automobile Insurance Coverage.
