What are the responsibilities and job description for the RN Admission Care Liaison FT Days/Evenings position at Ohio s Hospice Inc?
- The Admission Care Liaison is a DRIVING position supporting the Washington Courthouse, Wilmington, Fayette County area
- Schedule hours are M-F flexible hours, no weekends and 1-2 holidays per year

The RN Admission Care Liaison Essential Duties Are:
Ohio’s Hospice patients will receive quality nursing care and coordination of hospice services, when the RN/LPN Admissions Care Liaison admits the patient into the hospice program efficiently and provides the patient and family with the appropriate resources needed at the end of life. The RN/LPN Admissions Care Liaison will maintain a conversion rate of 75% and skillful use of the electronic medical record by accurate documentation at the point of care.
RN Admissions Care Liaison Qualifications:
RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred.
- CPR Certified
- Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime or inclement weather.
- Valid Driver's License with Safe Driving Record
- State Minimum Automobile Insurance Coverage
