What You Should Know About the RN Care Manager Role:
- The RN Care Manager is a DRIVING position and will require travel throughout Newark and Heath
- This is a Full-Time position, M-F 8A-430P with occasional on call weekend requirements and every 8th weekend required

The RN Care Manager's Essential Duties Are:
We provide superior care for patients in families in homes/facilities. The Care Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.
Qualifications:
· Active Registered Nurse license in the State of Ohio without any board actions.· Two years acute care nursing experience preferred
· CPR Certified
· Computer skills sufficient to properly document services and care.
· Ability to drive during daytime, nighttime or inclement weather.
· Valid Driver's License with Safe Driving Record
· State Minimum Automobile Insurance Coverage
