What are the responsibilities and job description for the RN/LPN Care Team Navigator Night shift position at Ohio s Hospice Inc?
- This position is FT, 6pm-6:30am with an every other weekend rotation and 2 holidays per year
- This position is an on site position located at our Care Coordination Center located in Centerville OH
Job Summary:
The Care Transition Navigator will ensure that Ohio’s Hospice patients and families receive excellent care and services. The Care Transition Navigator will coordinate care and provide support to patients, families, and Ohio’s Hospice teammates.
Essential Duties:
- Communicates with Medical Director and staff to collect accurate data
- Collaborates with caregivers/patients to determine needs
- Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio’s Hospice patients
- Participates in Continuous Quality Improvement activities to support the quality of Ohio’s Hospice services
- Uses appropriate customer skills to ensure effective communication
- Coordinates changes in the level of a patient’s care
- In an emergency or unusual circumstance will be expected to perform on-call visits to meet patient/caregiver needs
- The LPN will collaborate with a RN related to patient plan of care
- Precept new staff as requested
- RN develops and implements an individualized plan of care including creating measurable patient centered goals
- Collaborates and participates as a member of the care team
Qualifications:
RN’s - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
LPN's - Registered LPN with the State of Ohio without any board actions. One-year nursing experience preferred
Active Registered Nurse license in the State of Ohio without any board actions
Ability to provide direct patient care
Two years acute care nursing experience preferred
Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’s Hospice employees who wish to attain certification
CPR Certified
Computer skills sufficient to properly document services and care
Ability to drive during daytime, nighttime or inclement weather
Valid Driver's License with Safe Driving Record
