Health Care Delivery : Participates in our innovative healthcare delivery model that delivers high value care withunparalleled patient experience .
- Applies the nursing process to all areas of practice
- Establishes a patient plan of care; provides patient education related to patients condition, therapeutic regimen and self-care measures; evaluates effectiveness of patient education
- Provides care according to patient needs; performs specialized nursing care procedures as outlined in department / unit specific competency profiles
- Provides discharge planning that begins on admission according to patient need; coordinates patient care, admissions, transfers and discharges. Matching nurses with specific skills to patients with specific needs.
- Provides interdisciplinary communication to enhance the patient plan of care
- Enhances patient experience with high value care
- Actively participates in performance improvement initiatives positively affecting quality and safety

Minimum Qualifications
For Hire : Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program required.Current Ohio license or interim permit.
