About the Role:
As a Hospice Nurse Practitioner with our organization, you will be responsible for providing medical care, evaluating patient conditions, developing care plans, and prescribing medication. You will work under the supervision of a collaborating physician to ensure the best possible outcomes for our patients.
Key Responsibilities:
- Providing medical care and palliative treatment in accordance with state regulations
- Evaluating patient conditions through history and physical examinations
- Completing hospice face-to-face visit requirements to assess eligibility for certification and recertification
- Making recommendations to the hospice medical director regarding patient eligibility
- Ensuring accurate clinical notes and compliance with regulatory requirements
- Communicating with physicians and participating in interdisciplinary meetings to meet patient needs
Requirements:
- Graduate from an accredited School of Nursing and Nurse Practitioner program
- Current DEA license (if applicable) and nursing license in the state of practice
- CPR certification and Master's degree with a minimum of one year NP experience
- Hospice or palliative care experience is preferred, as well as certification in a specialist area
- Commitment to clinical and documentation excellence
