- Develop and maintain a service relationship with youth and family, and help to design and implement a service plan
- Provide youth with goals that help him/her to improve relationships with others and within the community
- Provide parents with information on disciplining, behavior management, household management and communication and relationship enhancement when needed.
- Works with the parents/caregivers so that they can support youth in achieving goals
- Provides crisis intervention and mediation as necessary
- Mentors will maintain a minimum productivity level of 85% billable contact per week
- Organization and maintenance of client information and case notes
- Recordkeeping and copying of necessary information, dissemination to appropriate individuals within the agency based on departmental requirements
- Participate in staff training, supervisory and staff meetings
- Maintains regular and reliable attendance
- Work is performed in the homes of clients and in the community. This may involve extensive driving with own car.
- English reading and writing skills required
- Interacts verbally with coworkers, representatives from outside agencies, and families
- Attention to detail and confidentiality required
- Works flexible hours
- Basic computer skills are required
Qualifications:
- High School Diploma/GED required
- Must be at least 21 years of age
- Must have a valid Ohio Driver’s License, proof of valid Auto Insurance and a safe driving record
- Bachelor's Degree in the social sciences preferred
- Experience working with special needs children is preferred.
