Description
Summary : Population Health Pharmacy Services (PHPS) is a division of OhioHealth Pharmacy Services that addresses the primary care needs of patients in our networks and in our direct-to-employer services.
The manager oversees a team of pharmacists and technicians that work with our patients, providers, and employers to improve the health of those we serve by optimizing ambulatory medication outcomes. The manager collaborates with the Primary Care Service Line to ensure that our associates are integrated into the ambulatory care experience and are working at the top of their licenses / certifications. The manager also works alongside the Population Health division to ensure that we are integrated into OhioHealth’s value-based care strategies and outcome measures related to HEDIS and CMS Stars.
Minimum Qualifications :

Registered Pharmacist (Ohio), Advanced degree (M. S. Hospital Pharmacy, M. B. A. ), preferred Hospital Pharmacy Residency or equivalent Experience preferred Business, Clinical Pharmacy, Performance improvement 5 yrs. in health care management.
