Job Description Summary
This position is responsible for assisting staff Physical Therapists in accordance with the Ohio Physical Therapy Board laws and rules. Following the set POC, the physical therapist assistant provides patient treatment, patient/family instruction and assists in discharge planning. The PTA documents in the medical record and communicates with the multidisciplinary team. The PTA collaborates with the supervising physical therapist including, but not limited to, the need for PT reassessment. This position helps patients achieve maximum functional outcomes. Acts as a clinical instructor for students and actively participates in department functions.

Physical Therapist Assistant Program or graduate of accredited PTA program. Licensed by the State of Ohio. Participation in student clinic rotations. in modalities to prevent injuries. Continuing in area of interest relevant to work assignment. New graduates with excellent references and/or one year rehabilitation Experience.
For this position, in the Home Care department only, Minimum Qualifications include: Valid state driver’s license, Minimum of eighteen (18) years old, Have an acceptable Motor Vehicle Report (MVR), Must provide proof of insurance, if applicable.

Direct Patient Care 90 - 100%
Treats Patients
- After Physical Therapist evaluation, treats patients based on plan of care and provides appropriate patient treatment techniques.
- Follows plan of care with constant communication with the Physical Therapist.
- Requests updates/revision to POC as needed.
- Responsible for maintaining/meeting the productivity expectation of their department.
- Instructs patient and care giver/family.
- Recommends DME required for discharge with discussion with Physical Therapist as needed.
- Provides information to physicians, nurses, case managers, social workers, and other team members.
- Involved in the multidisciplinary patient plan of care and discharge readiness plan,.
Indirect Patient Care 1 - 10%
Department Functions
- Supervises patient assignments and duties of the rehab techs.
- Is involved in department quality improvement through data collection and participation in team and staff meetings.
- Assumes duties delegated by management in the absence of department management, as needed.
