PRIMARY DUTIES AND RESPONSIBILITES

1.) Care Coordination:

  a) Provide outreach and engagement to families seeking WISe services (completing CANS assessment on

  new referrals within 7 days of the referral received.

  b) Formulation of the Child and Family Team and coordination of the team meetings.

  c) Develop and implement individualized plans with each youth and family that focuses on strengths and 

  needs.

  d) Frequent coordination with natural supports and cross system coordination.

  e) Coordinates monthly trainings and attends required collaborative meetings (FYSPRT, EnRoute).

  f) Provides education to community partners about the WISe program to promote community awareness

  and referrals to the program.

2.) Training/Coaching:

  a) Assists new staff in identifying required trainings and assures they have completed all mandatory 

  trainings required for WISe program. Monitors training manual and updates as needed.

  b) Provides supportive coaching to WISe family and youth peer partners as needed to support the WISe 

  program.

  c) Provides WISe staff with up-to-date direct service hours to assure WISe providers are meeting agency

  standards.

3.) Chart monitoring:

  a) Attends and successfully completes the QIRT training and has the ability to complete an internal audit on

  WISe charts as required on a yearly basis to assure compliance.

  b) Provides finance with an accurate count of enrolled WISe clients each month to assure the program is

  meeting agency standards.

4.) Clinical Documentation:

  a) Clinical documentation will be completed within 5 business days of the service.

 5.) Caseload Management:

  a) Maintain up-to-date documentations in accordance with WACs, RCWs and agency standards.

  b) Ongoing assessment of clinical need to determine frequency of sessions; must meet minimum standard as 

  a team of 10.5 direct service hours per family per month. 

6.) Attendance and Punctuality

  a) Ensures attendance and hours worked are accurately recorded in computerized timekeeping system

  b) Properly manages paid time off (PTO)

  c) Responsible for regular, predictable attendance and to work hours as scheduled, which may include

  evenings or weekends

Minimum Educational Requirements: 

  • Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, technical or health care related knowledge frequently acquired through completion of a trade school, para-professional, or certificate type program.
  • Is expected to learn and maintain higher level skills of the trade/profession, in addition to the basic training of the trade/profession.

Licenses & Certifications

  • Valid Washington State Driver's License
  • Agency Affiliated Certification:  State of Washington
  • Completion of, or ability to complete within 6 months of hire, Washington State Peer certification program (Certified Peer Counselor Licensure) is preferred if employee has a Bachelor's degree but required if employee does not.

Starting Salary: $23.08 - $24.81 per hour DOE. This is an hourly, non-exempt position.
