Oklahoma Behavioral Health in Kingston is seeking a BHCM II to join our team.
As a BHCM II you will work side by side with our team to help clients with referrals and/or connections to appropriate services, supportive counseling/education to promote or enhance community living skills, and fostering contacts with mentors, sponsors or others who can positively impact the clients' life within the community.
As a BHCMII you must have
- A Bachelor's or Master's Degree in Education or a Behavioral Health related field earned from a regionally accredited college or university recognized by the United States Department of Education (USDE)
Position Description
Under the direct supervision of the Clinic Director, assists with linking, advocacy, and referrals to needed services.
Responsibilities
1. Case Management services assure that clients are linked to needed services, regardless of where the services are offered. Services may include but are not limited to:
- Referrals and/or connections to appropriate service providers within the community.
- Supportive counseling/education to promote or enhance community living skills.
- Fostering contacts with mentors, sponsors, or others who can positively impact the client’s life within the community.
2. Maintain progress notes in an up-to-date manner per corporation guidelines.
3. Ensure progress notes reflect activities related to goals and objectives of approved treatment plan.
4. Select activities to meet individual and/or group needs.
5. Complete progress notes for each session and follow policy and procedure of corporation for placement in client files.
6. Maintain an environment that is functional and orderly.
7. Maintain a punctual and dependable schedule.
8. Demonstrate professional and ethical attitudes toward the corporation.
9. Communicate to the public the positive significance of mental health services.
10. Read and abide by policies and procedures which have been adopted by the corporation.
11. Work cooperatively with and be considerate of all corporation personnel.
