What are the responsibilities and job description for the Information & Referral Specialist / Social Worker position at Old Colony Elder Services?
OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, save lives for our consumers, we hope to allow them to stay as active and engaged in their lives and in their own communities for as long as possible.
To that end we are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.
What you will do :
- Initial and ongoing assessment of health and functional status of our members.
- Telephone and in person assessments.
- Develop community-based care plans and related service packages.
- Participate in the development of a member's Individual Care Plan (ICP)
- Arrange, coordinate, and monitor the provision of appropriate community services.
- Promote the independent functioning of members.
- Participate in the determination of a member's need for institutional and community long term care services.
