Old Colony Hospice and Palliative Care is a non-profit, 501c3 organization that has been providing end of life care in the community for over 40 years. We strive to provide outstanding quality of care and create a meaningful daily life for those living in our community. Our growing Palliative Care Consultation Program is seeking a Full-Time Nurse Practitioner to join our team. The Nurse Practitioner will serve as an integral part in delivering palliative care including but not limited to only Goals of Care conferencing, symptom management and end-of-life Care. Clinical practice will in the hospitals we serve. Our goal is to improve the quality of life in all the communities we serve by providing comprehensive chronic and palliative care to adults.
Responsibilities
· Establish relationships with existing hospital-based palliative care teams, partner companies, medical directors, hospital physicians, community physicians, nurses and other team members as well as community members to build a culture of collaboration and clinically integrated care across the continuum
· Be available to provide education and guidance to all persons providing patient care
· Participate in biweekly Palliative Care team meetings to review active cases
· Be available on an as needed basis to support other team members with clinical advice and assist with physician communication needs
· Prepare and maintain accurate patient records, charts and documents to support the medical practice and reimbursement for services provided
Requirements
· Current NP License
· One-year providing high-quality health care to a diverse patient population, preferably within an inpatient setting.
· Must have current and valid license issued by Commonwealth of MA; able to travel daily; pass pre-employment background, reference and drug testing
