Key Responsibilities:
- Assess, plan, and implement individualized treatment plans for patients in their homes.
- Provide therapeutic interventions to help patients improve or regain daily living skills.
- Collaborate with physicians, case managers, and other healthcare professionals to ensure coordinated, holistic patient care.
- Educate patients and families on how to enhance independence and improve their quality of life through therapy techniques.
- Document patient progress, treatment plans, and updates in accordance with regulatory standards.

- Ensure that all therapy practices comply with state and federal regulations.
- Valid and unrestricted Occupational Therapist (OT) license.

- Excellent communication and interpersonal skills.
- Ability to work independently while maintaining a collaborative approach with the healthcare team.
- Detail-oriented with excellent time management and organizational skills.
