About CommuniCare OLE
Established in 2023, CommuniCare OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY:
The Community Health Worker will be providing education, health promotion, outreach, and access to resources to patients and non-patients to increase health access in Solano County. Will also serve as the liaison between various community partners and CCOLE to build trust and awareness of the services we provide. The CHW will work in both the health center and community setting to reduce healthcare gaps in Napa County for disproportionately impacted communities; and help build trust and awareness about CommuniCare OLE. Major functions of this role will include educating the public on the services offered at CCOLE, coordinating community health events and assisting with connecting community members to services in and outside CommuniCare OLE.

DUTIES AND RESPONSIBILITIES
Serve as a liaison and bridge between community members and OLE Health. |
Under the direction of the Community Health and Outreach Manager, serves as lead successful execution of signature community health events and activities across Solano County, including but not limited to tabling, monthly food distributions in Napa and Calistoga, Dia de Muertos Celebrations, PRIDE events, and community health screenings. |
Travels extensively and drives Community Health vehicles to community locations and other destinations, including mobile health clinics, for community health activities and events at least four days per week – evenings, and weekend as needed |
Educates hard to reach vulnerable communities on how to access CCOLE’s services as well as advocacy to connect them to community resources and services |
Often represents organization at community partner events, including Chamber of Commerce, CBOs and others |
Builds and maintains excellent relationships with various stakeholders including patients/non- patients and hard to reach vulnerable populations, as well as community partners. |
Maintains, tracks, and reports on outreach program effectiveness after each outreach activity in the Salesforce database, making recommendations for improvement and identifying new opportunities. |
Provides information in a timely and ongoing manner to meet requirements for grant reporting. |
Develop and help maintain quarterly grant program reports. Supports the recruitment, management and recognition of community volunteers in Solano County. Actively alerts volunteers to upcoming opportunities, assists with onboarding of new volunteers and utilizes best practices in volunteer management including use of technology systems for scheduling and tracking volunteer activity and volunteer engagement and management. |

EDUCATION, EXPERIENCE, TRAINING
- High school diploma or its equivalent required
- Associate or bachelor's degree in Community Health, Human Services, or related field (Preferred)
- Covered CA and Cal Fresh enrollment certification (preferred)
- Strong verbal and written communication skills; Able to communicate fluently in English/Spanish or Tagalog (Preferred)
- Ability to communicate effectively with various stakeholders including patients, community partners, the community, and staff and management
- Strong computer and data entry skills preferred, as well as knowledge of Microsoft suite (Word, Excel, Power Point, Outlook, etc.)
- Ability to communicate effectively with patients, community, staff, and management
- Must have a minimum of 1 year of experience working, conducting, and coordinating community outreach/health promotion and education
- Customer service experience
- Must have experience working with marginalized/hard-to-reach populations
- Know and understand local health community needs, resources, and organizations to help support and improve health outcomes
- Critical, analytical, and evaluative thinking skills
- Ability to manage multiple tasks independently and as part of a team
- Valid CA Driver’s License required
