PAY RANGE:                      $62,878.40 to $76,835.20 annually

JOB SUMMARY: This position reports to the Director of Nursing and works as part of a team with clinical staff and Human Resources to provide quality, coordinated immunization care to patients and employees, as well as employee-related health and safety practices.


EDUCATION, EXPERIENCE, TRAINING

  1. Completion of an accredited LVN Program.
  2. Current California LVN license and good standing with the Board of Vocational Nursing required.
  3. Current CPR certification required. 
  4. Bilingual Spanish/English preferred.

 

KNOWLEDGE OF WORK

 

1.

Knowledge of nursing processes, scope of practice and standards of care.

2.

Previous nursing experience in a medical office, outpatient clinic or hospital strongly preferred.

3.

Previous experience working with continuous quality improvement processes and with safety, environmental and/or infection control standards strongly preferred.

4.

Demonstrated ability to assume responsibility, to work well on a team and problem-solve.

 

DUTIES AND RESPONSIBILITIES

 

1.

Provide agency-wide coordination and oversight of immunization services and ensure compliance activities are met as outlined under the Vaccines for Children (VFC), Vaccines for Adults (VFA) (317 Program) and CA Immunization Registry (CAIR) program guidelines. Key activities: 

  1. Maintain and update the Routine Vaccine Management Plan and the Emergency Vaccine Management Plan, and train key clinic staff in the emergency plan.
  2. Coordinate and submit Annual VFC and VFA Recertification. 
  3. Coordinate, prepare and facilitate annual state site visits.
  4. Coordinate and conduct VFC and VFA program trainings for Medical Assistants.
  5. Assist clinical staff in monitoring and maintaining vaccine transportation supplies and train key clinic staff in proper vaccine transport.
  6. Work with Facilities Director to ensure proper maintenance of vaccine storage.
  7. Collect and prepare data indicators to ensure program compliance required by VFC, FVA, and CAIR.
  8. Work with clinical staff at all sites to ensure providers and other medical staff have up-to-date knowledge of current health and immunization standards.
  9. Monitor ongoing changes and updates as required by VFC, VFA and CAIR.
  10. Coordinate and ensure appropriate storage compliance for purchased vaccines. 

 

2.

Coordinate vaccination activities for employees and patients. Assist Clinic Managers in ensuring vaccine inventory is assessed, ordered, monitored, and returned on time.

3.

Administer vaccines.

4.

Work with Human Resources and Clinic Managers to coordinate annual TB testing and complete tracking for all employees.

5.

Work with clinic staff to ensure appropriate storage compliance of miscellaneous items located in refrigerator and freezer units. Miscellaneous items may include but are not limited to: Insulin, A1C cartridges, pharmaceutical samples. 

6.

Establish and maintain a collaborative working relationship with Yolo County Health and Human Services Agency’s immunization employees and attend coordination meetings, as needed.

7.

Develop and participate in other activities to promote and increase timely immunizations. 

8.

Provide ongoing education and training for clinical staff on immunization administration.

9.

Perform initial and annual N–95 Mask Fit Testing for employees.

10.

Manage and track employee COVID-19-related absences.

11.

Investigate employee work-related injuries and accidents in partnership with Human Resources, as necessary.

12.

Present work-related safety presentations to staff.

13.

Perform ergonomic evaluations for staff in partnership with Human Resources and recommend appropriate corrective measures. 

14.

Performs patient care activities in primary care clinics within the licensed vocational nursing scope of practice as needed. 

15.

Process clinical patient information for providers in patient electronic health records (EHR).

16.

Participate routinely in quality improvement projects using PDSA model.

17.

Assist with coordination of care for other CommuniCare services, including dental care, behavioral health, diabetes education, etc. 

18.

Attend clinic and general CommuniCare OLE meetings as required. 

19.

Other duties as assigned.
