POSITION TITLE:  Referral Specialist - Yolo (Will sit in West Fairfield)

REPORTS TO:       Referral Coordination Manager       

SUPERVISION GIVEN: None

LOCATION: West Fairfield, CA

HOURS: 40 hours a week, Mon to Fri

PAY RANGE: $25.68 To $31.39 Hourly

Established in 2023, CommuniCare OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Referral Specialist is a member of the CommuniCare OLE clinical care team and works in continuous partnership with the patient, their family when appropriate, CommuniCare OLE care team, and relevant community partners to improve health outcomes and meet the patient’s desired health and wellness goals by:

  1. Serving as the contact point, advocate, and informational resource for patients, family/caregiver(s), primary care team, payers, and community resources
  2. Facilitating access to internal and external services
  3. Identifying and addressing gaps in care
  4. Increasing patient’s ability for self-management and shared decision-making

 

KNOWLEDGE OF WORK

Knowledge of Organizational Improvement Methods
Knowledge care coordination best practices
Knowledge of Federally Qualified Health Centers and the key health center principles and guidelines

 

DUTIES AND RESPONSIBILITIES

Supports and collaborates with primary care provider by processing referrals to specialty care providers and communicating and documenting barriers to that care.
Ensures closed-loop patient referrals (both internal and external), including documentation and data maintenance.
Facilitates patient access to appropriate medical and specialty providers
Communicates updates regarding specialty provider access to direct supervisor.
Processes initial consult authorization to specialty office.
Schedules patient transportation to specialty provider appointment, if needed
Schedules translation services for the specialty appointment, if needed
Documents test result data correctly and consistently in the EMR
Collaborates with care coordinators in processing live-medical referrals
Documents timely and efficiently in EMR
Attends appropriate training courses/webinars and meetings
Provides feedback for the improvement of referral coordination processes
Performs other duties as assigned

 

EDUCATION, EXPERIENCE, TRAINING

  1. High School Diploma or equivalent.
  2. Experience working with diverse populations; displays respect and sensitivity to cultural diversity.
  3. Experience working in community health setting.
  4. Strong collaborative skills; ability to effectively and appropriately engage members of the care team, other clinic departments, leadership, and external community partners.
  5. Exceptional communication skills; ability to effectively engage patients and assess needs and concerns to ensure that the patient is referred appropriately.
  6. Excellent in-person and phone customer service skills; able to manage difficult and emotional patient situations and to respond promptly to patient needs.
  7. Ability to adapt to change; approach challenges with an innovative and proactive attitude; and investigate resources.
  8. Strong verbal and written communication skills required; Bilingual (English/Spanish) required.
  9. Must certify and remain current in CPR.

CommuniCare OLE requires pre-employment drug-testing (4-panel drug-test includes: amphetamines, cocaine, opiates, and PCP).
