Make a difference in the lives of at-risk children and families with the School Based Clinical Team. Longstanding FQHC is seeking a Licensed Clinical Social Worker to join their School Based Health Center team. The School Based Clinician will complete psychosocial intake & ongoing assessments. Complete person-centered treatment plans prioritizing goals & objectives. Provide office-based individual / family and / or group therapy with children and / or adults presenting with mental health and / or substance challenges. Participate in Interdisciplinary Team Meetings as well as other pertinent meetings / appointments to ensure all team members are continuing open communication of the individuals progress. Collaborate with other providers and / or community resources to support community mental health needs. Support and complete step-down / aftercare / discharge plans. Full or Part-time.
