What are the responsibilities and job description for the Care Coordinator (WISe Program - Tacoma) position at Olive Crest?
POSITION SUMMARY
The Care Coordinator will work as a part of the Wraparound with Intensive Services (WISe) team to help implement mental health and wraparound services to youth and families being primarily responsible for case management, care coordination, partnering with community professionals and natural supports, aiding the therapist in the implementation of the safety and treatment plans, and facilitating child and family team meetings.
JOB DUTIES & RESPONSIBILITIES
- Communicate and explain WISe services and the WISe process including the essential phases and activities with youth/family.
- Summarize agency protocols and legal/ethical mandates with youth/family and issues to team members.
- Support the team in identifying the team mission and adhering to the WISe process.
- Provide culturally relevant skill building, individual and family therapy.
- Be able to support youth and families comfortably and without judgement in home and community settings.
- Maintain a caseload of approximately 8-12 children/families.
- Maintaining enthusiasm and optimism for services and participation in the WISe process
- Provide collateral information to the care coordinators for the CANS and support the care coordinator with orienting families to CANS.
- Maintain CANS rater certification to ensure understanding of the CANS.
- Coordinate and facilitate child and family team meetings with team members.
- Partner with the Family Therapist in the development of a safety/crisis plan, discuss immediate crisis with youth/family and assist stabilizing dangerous or harmful situations.
- Ability to recognize child and family strengths and how to strategically utilize these to facilitate growth within the child and family while ensuring that these strengths are utilized throughout the therapeutic process.
- Participate in weekly meetings including program staff meetings and trainings.
- Respond to and attend to details of crisis situations that may arise during working and non-working (on-call) hours.
- Complete documentation and trainings as assigned.
- Other duties as assigned.
REQUIRED QUALIFICATIONS
- Bachelor’s Degree from an accredited college or university in one of the following: Social work, social welfare, marriage, family and child counseling, child development, counseling psychology, social psychology, clinical psychology, educational psychology, human services, behavioral sciences or related field.
- Become an Agency Affiliated Counselor upon hire.
- Computer skills. Comfortable working with online documents, databases and other web-based applications.
- Record clearance free of criminal conviction.
- Reliable transportation and willingness to drive between sites for intakes and various community meetings.
- A philosophy of support for at-risk children and families in line with the mission, vision, values, and statement of faith of Olive Crest.
DESIRED QUALIFICATIONS
- At least 2 years of experience previously as a Case Manager.
- Experience in a culturally diverse workplace, working with at-risk children and families and/or child welfare, or in a non-profit organization.
