Olive Grove Hospice is seeking a dedicated and compassionate Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will work closely with patients and their families to provide high-quality care and support.
Your Responsibilities:
Develop and implement individualized care plans that meet the unique needs of each patient. Coordinate with healthcare providers to ensure seamless care transitions. Provide emotional support and education to patients and their families. Participate in interdisciplinary team meetings to discuss patient progress and develop care plans. Maintain accurate and up-to-date records of patient care. Collaborate with other healthcare professionals to ensure continuity of care.
Requirements:
Graduate of an accredited school of nursing, Registered and licensed with the State of working as a RN, Current unencumbered license with the Board of Nursing, Experience within a long term care facility or homecare or hospice preferred, Excellent judgment and decision-making abilities, Ability to deal tactfully and use good communication and customer service skills with staff, residents and their families, Basic computer skills and ability to use electronic records, Must pass criminal background from Department of Human Services at the time of hire and every 4 years after, Must be screened for and pass TB test, Must pass a drug screening test.
