We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Olive Grove Hospice. As a Hospice RN Case Manager, you will play a vital role in coordinating care for our patients and their families.
Your Responsibilities:
Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. Coordinate patient services with other members of the IDG as well as contracted services when needed. Consult with the attending physician concerning changes in patients' condition requiring order changes and/or changes in the plan of care. Participate in IDG and case conferences. Take responsibility for ensuring patients' plans of care are kept current. Perform on-call duties per policy and agency need. Conduct initial as well as ongoing comprehensive assessments of the patients. Supervise and evaluate the care given by the Hospice Aide at a minimum of once every 14 days and as needed.
Requirements:
Graduate of an accredited school of nursing, Registered and licensed with the State of working as a RN, Current unencumbered license with the Board of Nursing, Experience within a long term care facility or homecare or hospice preferred, Excellent judgment and decision-making abilities, Ability to deal tactfully and use good communication and customer service skills with staff, residents and their families, Basic computer skills and ability to use electronic records, Must pass criminal background from Department of Human Services at the time of hire and every 4 years after, Must be screened for and pass TB test, Must pass a drug screening test.
