About Us:
Olive Grove Hospice provides compassionate care to our patients and their families. We are seeking a skilled Hospice RN Case Manager to join our team.
Our Hospice RN Case Manager will be responsible for coordinating patient services, evaluating the effectiveness of hospice services, consulting with physicians, submitting clinical notes, participating in case conferences, performing on-call duties, conducting comprehensive assessments, supervising Hospice Aides, and maintaining current licenses. This role requires excellent judgment and decision-making skills. The ideal candidate will have experience in long term care or homecare/hospice settings.
Qualifications:
Graduate of an accredited school of nursing, Registered and licensed with the State of working as a RN, Current unencumbered license with the Board of Nursing, Experience within a long term care facility or homecare or hospice preferred, Excellent judgment and decision-making abilities, Ability to deal tactfully and use good communication and customer service skills with staff, residents and their families, Basic computer skills and ability to use electronic records, Must pass criminal background from Department of Human Services at the time of hire and every 4 years after, Must be screened for and pass TB test, Must pass a drug screening test.
