Job Description:

We are currently seeking a Nurse Practitioner to provide medical services to patients living in long-term care facilities. We are looking for a nurse practitioner who has experience working with long term patients. The perfect candidate is well rounded in knowledge, experience and filled with a positive personality. The candidate must be able to work well individually as well as in a team. We are looking for someone who has a sincere passion about providing the appropriate health services and giving the best care possible. Please read below to see some of the responsibilities of a NP.

Nurse Practitioner Job Duties:

· Provide primary and preventative care for long-term care patients residing in skilled nursing facilities.

• Responsible for ensuring that all details, diagnosis and observances will be documented in the resident’s medical record.
• Evaluate and manage residents with acute change in condition in a timely manner.
• Prescribe appropriate medications, including controlled substances in accordance with state laws and patient diagnosis.
• Order, review, and/or follow-up with treatments and diagnostic testing results.
• Monitors and schedules patient care visits from electronic resources such as EMR, Dashboards and Telehealth when necessary.
• Completes and submits daily billing log, includes CPT codes and ICD 10 codes for resident care visits.
• Communicate with the nursing home staff and primary care physician on a regular and as needed to discuss changes in condition and/or the care plan.
• Complete all clinical documentation and assignments in accordance to facility/company’s expectations and protocols in a timely manner.

REQUIRED Licensures/Certificates:

· NP Medical License in the appropriate state that you will be working in

· Board Certification (such as ANCC, PNCB, etc)

· DEA Certificate

· Malpractice Certificate- Must be Active

· NPI

· ACLS/BLS Certificate

· Accredited School Degree

· Infection Control Certificate

Looking For a Candidate that has:

· Clinical background in adult, family, or geriatric specialties.

· Experience working in a long-term care setting.

· Able to work with an EMR.

· Can manage patients with advanced illness and multiple chronic conditions.

· Capability to work with an interdisciplinary care team to coordinate patient care.
