Bachelor's degree from an accredited college or university in social work, psychology, sociology, or a closely related field.
OR
Bachelor's or Master's degree from an accredited college or university and one (1) year of experience as a social worker, case manager, or care coordinator in a public or private social services agency.Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver's license and be free of any major traffic violations for the last three (3) years.
Desirable Qualifications:
Two years of social service-related experience.
This position provides assessment, consultation and referral and collaboration services to individuals, families, and target groups in the community. Works primarily within the Adult & Family Services Division. Work assignments include carrying a caseload of members and providing care coordination services for managed care organization (health plan) contracts for individuals 18-64 years of age. Supports a diverse, respectful, and inclusive workplace.
- Provides assessment and care coordination for individuals 18-64.
- Interviews and assesses client to determine and identify problems or needs for service/ programs.
- Develops and writes individual service plans.
- Provides information about health care access and assists individuals to access Medicare and Medical Assistance funded health care services.
- Provides information about managed care health benefits.
- Completes required contacts and paperwork within established timelines.
- Supports and informs clients of community resources, information and referrals for services, rights and responsibilities under programs or services.
- Performs related duties as assigned.
- Makes frequent phone calls to actively engage them in care coordination.
- Considerable knowledge of the social sciences
- Knowledge of home and community-based services and supports.
- Knowledge of social and economic problems
- Knowledge of motivating factors of human behavior
- Knowledge of the principles of social work
- Knowledge of community resources
- Knowledge of federal and state welfare programs and relevant laws
