Responsible to: Director of Residential Services
Job Summary: The Team Lead is an integral part of the treatment team in the site. The Team Lead is responsible for assisting the RM and ASM with supporting, coaching, mentoring and directing the Direct Support Professionals (DSP) to ensure the general care of each individual occurs and all OLV documentation pertaining to the individual is completed per OPWDD regulations.
Essential Job Duties:
Strength: Sufficient to assist with lifting and transferring a patient and perform CPR.
Mobility: Sufficient mobility to bend, stoop, and bend down to the floor, ability to move around rapidly; and to move in small, confined areas.
Hearing: Sufficient to hear through the stethoscope to discriminate sounds; to hear cries for help; to hear alarms on equipment and emergency signals; and various pages.
Vision: Sufficient to make physical assessment of patients and equipment.
Communication: Able to communicate in both verbal and written formats and interact with clients, staff and supervisors.

Job Summary: The Team Lead is an integral part of the treatment team in the site. The Team Lead is responsible for assisting the RM and ASM with supporting, coaching, mentoring and directing the Direct Support Professionals (DSP) to ensure the general care of each individual occurs and all OLV documentation pertaining to the individual is completed per OPWDD regulations.
Essential Job Duties:
- Initiate a "Shift Update” to enhance communication and RICH Relationships at the site.
- Provides positive role model to DSPs, of the expectations and culture of accountability for the site.
- Proficient knowledge in the DD program and Life Skills areas.
- Initiates and assists the DSPs with the completion of individual assignments and ensures supervision levels are followed at the site.
- Ensures Behavior Support Plans are followed as written, and all documentation has been completed per regulation.
- Ensures and initials that DSPs maintain meaningful documentation records for each individual (including but not limited to; intake and output, medical monitoring, habilitative program documentation, Therap notes, behavior notes/documentation, incident reports, etc).
- Ensures in-house recreational activities and community recreational activities occur as planned.
- Assists with training, coaching, and supporting DSPs, as necessary.
- Float to department sites as needed.
- Participates in the weekly site team meeting to ensure ongoing communication with the RN Case Manager/PC/RM/ARM.
- Attends department staff meetings, division management meetings, and or other meetings as requested.
- Assists with Administering medications to the individuals, as necessary.
- Completes meal observation audits at assigned IRAs.

- 3 to 5 years applicable prior experience with HS diploma or 2 years with an Associate’s Degree
