Consistently communicates with CFY Supervisor regarding mandated CFY paperwork, observations and documentation set forth by ASHA, NYS and Erie County2. Cooperatively accepts feedback and suggestions form CFY Supervisor3. Reviews all available documentation related to the individuals serviced prior to beginning treatment, including IEP/IFSP, script and prior evaluations if available4. Performs treatments according to the IEP/IFSP and accepted professional standards and practices5. Conducts ongoing assessment of individuals during each visit and modifies goals/outcomes/treatment plans accordingly6. Performs appropriate parent/caregiver education and develops home programs as necessary7. Works as part of an inter-disciplinary team to enhance treatment program of the individual by utilizing communication with all team members8. Maintains an accurate schedule9. Submits all required documentation of visits according to established procedures10. Communicates with families on a regular basis11. Keeps abreast of legal responsibilities, professional ethics, standards of practice and agency policies and regulations12. Discontinues therapy when an  individual no longer meets eligibility requirements13. Provides own transportation to service locations14. Maintains confidentiality with HIPPA/FERPA and agency guidelines15. Maintains professional appearance by following agency dress code16. Attends department meetings and any other meetings required by supervisor17. Adheres to and signs Annual Employee Agreement18. Documents time off and time worked accurately according to agency policies19. Performs other duties and assumes other responsibilities as assigned by direct supervisor  Skills:• Basic computer skills  Competencies: • Strong written and verbal communication skills• Strong organizational skills• Strong time management skills• Good interpersonal skills Minimum Degree Required:  Master’s Degree in Speech/Language Pathology/Communication Disorders Minimum Experience Required: Less than one year Required License/Registration/ Certification: None Physical Requirements: The frequent physical demands of the position are sitting, standing, walking, climbing, talking, listening, repetitive motions , lifting, carrying, pushing, pulling, stooping, balancing, crouching, reaching, handling, and grasping of children.

Salary : $180 - $51,250
