The Transition Specialist provides support and training for individuals to be as independent and productive as possible in their work life and to be meaningfully integrated into the community, including transition skills to gain job skills, employment and independent living.Essential Job Duties:1. Provide Transition Lessons and activities related to transition in classroom.2. Administer Vocational Assessments and Interest Inventories. 3. Develop Training Plans and Employability Profiles with students.4. Develop collaboration with Community partners to provide career exploration and work-based learning upon youth’s release from detention.5. Schedule students for vocational experiences. 6. Coordinate with community partners to provide career exploration and work-based learning. 7. Complete documentation and track WBL hours necessary to meet NYSED requirements.Skills: • Computer skills• Interpersonal skills• Conflict resolution• Case managementMinimum Education Required: • Bachelor Degree (Master`s Degree in Special Education preferred)Minimum Experience Required: • 1-2 years working with individuals who have developmental disabilities and/or mental health concerns.Required License/Registration/ Certification:• Valid NYS Driver’s licenseTraining:• Continuing education as per NYS Office of Profession guidelines• All required agency trainings (including Applied Behavior Analysis, SCIP-R)• All trainings required by programs in which you service individualsPhysical Requirements:The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required. There is little, if any, exposure to environmental conditions.• The constant physical demand of the position is sitting.• The frequent physical demands of the position are standing, walking, climbing, talking, hearing, repetitive motions, and eye/hand/foot coordination. • The occasional physical demands of the position are lifting, carrying, pushing, pulling, balancing, stooping, crouching, reaching, handling, grasping, and feeling.
