POSITION SUMMARY
Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment; work cooperatively with the patient, and family; and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.
POSITION DUTIES AND RESPONSIBILITIES
- Proficient in offering direct care to patients based on the guidelines set by the RN and in alignment with the Aide Plan of Care, ensuring that each patient's unique needs are met with utmost precision and empathy.
- Possess adeptness in safely transferring and ambulating patients, adhering strictly to the protocols established by the Hospice agency.
- Demonstrates the ability to accurately document care details, ensuring all pertinent information is recorded. Has a keen sense for recognizing and promptly reporting any changes in the patient's condition.
- Upholds stringent infection control standards, maintaining sanitation and cleanliness. Ensures the patient's environment is clean, safe, and conducive to their well-being.
- Emphasizes open, positive, and respectful communication, ensuring that both patients and their families feel supported and informed at all times.
- Holds a deep respect for patient autonomy, ensuring their privacy is protected and their property respected.
- Equipped with the know-how to aid patients in maintaining proper nutrition and ensuring they receive adequate fluid intake, vital for their well-being.
- Contributes to the smooth functioning of the Hospice agency, demonstrating an ability to manage time efficiently and prioritize tasks effectively.
- Trained and skilled in executing various treatments and procedures in line with the Hospice agency's established policies.

POSITION SKILLS AND QUALIFICATION
- High School Diploma Preferred.
- Has successfully completed one of the following:
- Training program/competency evaluation as specified in §418.76(b)(c) of the Medicare regulations;
- Competency evaluation program that meets §418.76( c);
- Nurse aide training/competency evaluation program approved by the state and listed in good standing on the state nurse aide registry;
- State Licensure program that meets requirements of §418.76(b)(c).
- Must have a current driver’s license.
- At least one year experience preferred as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long-term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program.
- Must be able to read, write, and verbally report clinical information to patients, representatives, and care givers, as well as to other hospice staff. Demonstrates interest in the welfare of the ill and elderly. Successful completion of the Aide Competency Evaluation Skills Checklist that meets §418.76(c) and written Skills Test.
- Should be CPR certified.
- Reliable transportation and valid auto liability insurance.
- Microsoft Office Suite Proficiency: Word and Excel.
- Good interpersonal skill.
- Familiarity with platforms like Wellsky or Consolo to maintain and access patient information, care plans, medication histories, and more.
