Omni Wound Physicians (OWP) is hiring Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile wound care services in skilled nursing facilities, long-term care centers, and private residences throughout California.
This is a dynamic, field-based role ideal for Advanced Practice Providers (APPs) who are clinically focused, organized, and dedicated to improving patient outcomes through hands-on wound care.
Hiring Areas

Central California – Fresno, Bakersfield
Northern California – San Francisco, San Jose, Sacramento

Schedule & Shift Details
Minimum scheduling: At least two shifts per work week
Shifts typically between 7:00 AM – 7:00 PM local time
Full-time: 30 hours/week
Part-time: Minimum two shifts/week
On-call or weekend coverage may be required with advance notice
Employment classification is based on scheduled hours, patient volume, and market demand
Providers may structure routes to optimize travel and visit efficiency within assigned regions

Field Assistant Support:
Assigned based on market needs, patient volume, and provider workload. Field Assistants assist with documentation, wound photography, logistics, and coordination to support care quality and workflow.
Key Responsibilities
Evaluate, diagnose, and manage a variety of wounds (pressure ulcers, diabetic wounds, surgical wounds, venous/arterial ulcers)
Perform procedures such as sharp debridement, tissue biopsies, NPWT, and dressing changes
Create and adjust wound care treatment plans
Educate patients, families, and facility staff on wound care and prevention
Maintain complete, HIPAA-compliant documentation using secure EMR
Drive between patient locations as required; mileage may vary and may exceed 50–70 miles per day depending on service area and home location
Perform other reasonable duties as assigned
Qualifications & Experience
Required:
Graduate of an accredited NP or PA program
Active, unrestricted license in California
National board certification (AANP, ANCC, or NCCPA)
Minimum 1 year of clinical experience (wound care, geriatrics, internal medicine, or similar)
BLS certification
Valid California driver’s license and reliable transportation
Preferred:
Wound care certification (CWSP, CWS) or willingness to obtain within 6 months
DEA registration – not required but preferred
Experience in skilled nursing, home health, or mobile care settings
Proficiency in EMR and digital documentation systems
Ideal Candidate Profile
The ideal APP is clinical, adaptable, and patient-focused. They have strong critical thinking and decision-making skills, maintain excellent documentation practices, and are comfortable working independently across various care settings. They are proactive, organized, and skilled at building rapport with patients and facility staff. A background or strong interest in wound care is highly valued.
Regulatory & Compliance – California
Nurse Practitioners:
Active CA NP license
National certification (AANP or ANCC)
Furnishing number and prescriptive authority agreement (as applicable)
DEA registration – preferred but not required
30 CE hours every two years
Physician Assistants:
Active CA PA license
National certification (NCCPA)
Delegation of Services Agreement (DSA) with supervising physician
DEA registration – preferred but not required
50 CME hours every two years
All providers must remain compliant with the California Board of Registered Nursing or the California Physician Assistant Board.
Physical, Cognitive & Technical Requirements
This mobile care role requires the ability to walk, stand, kneel, bend, lift up to 50 pounds, and safely operate a motor vehicle to travel between care sites. Cognitive abilities such as clinical judgment, focus, and multitasking are essential. Proficiency in using mobile devices, EMRs, email, and team communication platforms (e.g., Slack) is required for care coordination and documentation.
