Essential Responsibilities Include :

• Chronic wound management : Evaluation and assessment, diagnosis, treatment, including debridement of wounds and application of appropriate dressings.
• Gastric feeding tube replacement services.
• Communication with the patient’s PCP, nursing staff, patient, and family members.
