Job Title: Case Manager - Home Visits for Adult and Foster Care

Job Summary: We are seeking a dedicated Case Manager to provide comprehensive support and coordination for adult and foster care members through home visits. This role involves assessing individual needs, developing care plans, and ensuring access to necessary resources and services. The ideal candidate will possess strong interpersonal skills and a commitment to enhancing the well-being of vulnerable populations.

Key Responsibilities:

  • Conduct home visits to assess the needs of adult and foster care members.
  • Develop and implement individualized care plans in collaboration with members and their families.
  • Coordinate services and resources, including medical, social, and community support.
  • Monitor progress and adjust care plans as needed.
  • Maintain accurate records and documentation of interactions and services provided.
  • Advocate for members’ needs and rights within the healthcare and social service systems.
  • Collaborate with multidisciplinary teams to ensure holistic care.

Qualifications:

  • Bachelor’s degree in social work, nursing, or a related field; licensure preferred.
  • Experience in case management, preferably with adult or foster care populations.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Knowledge of community resources and services.

Job Types: Full-time, Contract

Pay: $20.00 - $25.00 per hour

Schedule:

  • Monday to Friday
